As to copyright notice, I am unfamiliar with the laws of India in these things. If there is a way of making proper registration there, it should be done. You might investigate this with profit. How are other of your books handled? If there is no protection available there you might do as you suggest stating that "This work is published with the permission of Ellen G. White Estate, Incorporated, the holders of the copyrights."
